乳腺浸润性癌中E2F1和P16蛋白的表达及意义

摘    要:目的 探讨转录因子E2F1和P16蛋白在乳腺浸润性癌中的表达及其对乳腺癌发生、发展及预后评估的意义,并为乳腺癌的靶向治疗提供一定的依据.方法 采用免疫组织化学方法检测61例乳腺浸润性癌、49例导管内乳头状瘤及58例乳腺增生组织中E2F1和P16蛋白的表达情况.结果 E2F1在乳腺浸润性癌、导管内乳头状瘤及乳腺增生组织中的阳性率分别为78.7%、12.2%和1.7%,前者与后二者分别比较差异有统计学意义(P<0.05);P16在乳腺浸润性癌、导管内乳头状瘤及乳腺增生组织中的阳性率分别为34.4%、57.1%和70.7%,前者与后二者分别比较差异有统计学意义(P<0.05).癌组织中E2F1和P16阳性表达与患者年龄及肿瘤TNM分期差异无统计学意义(P>0.05),与淋巴结转移及组织学分级差异有统计学意义(P<0.05),E2F1和P16蛋白在癌组织中的表达差异有统计学意义(P<0.05);相关性分析显示,呈显著负相关性(r=-0.297,P<0.05).结论 E2F1、P16蛋白可能参与了乳腺癌的发生和发展,E2F1蛋白的阳性表达与P16失表达可能是患者预后差的指标,二者联合检测更有利于评估预后,同时可为乳腺癌新的生物靶向治疗提供依据.

Abstract:Objective To study the expression of E2F1 and P16 protein in invasive breast cancer and its significance to the occurrence,progress and prognosis of breast cancer,and to provide the theoretical basis for its targeted therapy.Methods The expression of E2F1 and P16 protein was detected by the immunohistochemical method in 61 cases of invasive breast cancer,49 cases of intraductal papilloma and 58 cases of breast hyperplasia.Results The positive expression rate of E2F1 was 78.7%,12.2% and 1.7% in invasive breast cancer,intraductal papilloma and breast hyperplasia,respectively,there was statistical difference between invasive breast cancer with the others(P<0.05).The positive rate of P16 expression was 34.4%,57.1% and 70.7% in invasive breast cancer,intraductal papilloma and breast hyperplasia,respectively,and there was statistical difference between invasive breast cancer with the others(P<0.05).The expression of E2F1 and p16 protein was not re1ated with age,TNM classification of tumor(P>0.05),and positively correlated with axillary lymphatic metastasis and histologic grading(P<0.05).The expression of E2F1 and P16 was significant negatively correlated in invasive breast cancer(r=-0.297,P<0.05).Conclusion The expression of E2F1 and P16 may be involved the occurrence and progress in invasive breast cancer.The positive expression of E2F1 and the loss expression of P16 would be the markers of poor prognosis of patients.Their combination detection detected may be more conducive to evaluate the prognosis and would provide the theoretical basis for the biological targeted therapy of breast cancer.
