甲状腺功能异常患者血清FGF21水平及其影响因素

目的检测不同甲状腺功能状态下人体血清成纤维细胞生长因子21（FGF21）水平的差异,探讨影响FGF21的水平的因素。方法本实验按受试者甲状腺功能状态分为甲亢组（67例）、甲减组（48例）、对照组（53例）,分别检测血清游离三碘甲状腺原氨酸、游离甲状腺素、促甲状腺激素、FGF21、三酰甘油、总胆固醇、高密度脂蛋白、低密度脂蛋白、载脂蛋白A1、载脂蛋白B、总蛋白,并进行统计分析。结果①甲亢组血清FGF21明显低于对照组（P〈0.01）;甲减组血清FGF21高于对照组（P〈0.01）。②相关性分析显示,甲亢组血清FGF21水平与促甲状腺激素、三酰甘油呈正相关（r=0.294,P〈0.05;r=0.267,P〈0.05）,与年龄呈负相关（r=-0.312,P〈0.01）;甲减患者血清FGF21与促甲状腺激素、三酰甘油、低密度脂蛋白呈正相关（r=0.832,P〈0.01;r=0.381,P〈0.05;r=0.373,P〈0.05）。③多元线性回归分析显示,甲亢组中,年龄和TSH是影响血清成纤维细胞生长因子21的独立因素;甲减组中,仅TSH是影响血清FGF21的独立因素。结论甲亢组中血清FGF21浓度明显降低,甲减组中血清FGF21浓度明显升高;年龄、促甲状腺激素可影响血清FGF21水平。

Changes and its influence factor of serum fibroblast growth factor 21 levels in patients with thyroid dysfunction

Obiective To explore the changes and its influence factors of serum fibroblast growth factor 21 levels in hyperthyroid and hypothyroidiam.Methods Free-triiodothyronine,free-thyroxin,thyrotrophin,FGF21,triglyceride,total cholestero,high density lipoprote in-cholesterol,lowdensity lipoprote in-cholesterol,apolipoprotein A1,apolipoprotein B and total protein of(67 cases) patients with hyperthyroid,(48 cases) hypothyroidiam and(53 cases) healthy subjects were measured.All the people in the experiment were diagnosed by their thyroid function.Serum FGF21 levels in hyperthyroid and hypothyroidiam were compared with the healthy controls,and the influence factors of serum FGF21 were analyzed statistically.Results ①It was found that FGF21 in hyperthyroid were significantly lower than that in healthy subjects(P < 0.01);the serum levels of FGF21 in group of hypothyroidism were higher than those in control groups(P < 0.01).②The serum levels of FGF21 in hyperthyroid was negatively related with age(r =-0.312,P < 0.01) and positively related with TSH,TG(r = 0.294,P < 0.05;r = 0.267,P < 0.05);the serum levels of FGF21 in hypothyroidism was positively related with TSH,TC,LDL(r = 0.832,P < 0.01;r = 0.381,P < 0.05;r = 0.373,P < 0.05).③Multiple linear regression analysis showed that in hyperthyroid,age and TSH were independent factors influencing serum FGF21 levels.In hypothyroidism only TSH is the independent factor influencing serum FGF21 levels.Conclusion Serum FGF21 levels in hyperthyroid are decreased,but in hypothyroidism are increased.Age and thyrotrophin can influence serum FGF21 levels.
