超敏肌钙蛋白T对急性冠状动脉综合征的临床诊断及短期预后价值

目的探讨血清超敏肌钙蛋白T(hs-TnT)对急性冠状动脉综合征(ACS)患者的临床诊断及短期预后价值。方法选择2010年1月～2011年6月本院收治的急性冠状动脉综合征患者186例,所有患者入院后24 h内做血清hs-TnT检测及常规TnT检测,住院期间做冠状动脉造影,比较hs-TnT及常规TnT检测对心肌坏死的检出率,分析hs-TnT水平与冠状动脉病变程度及范围的关系。随访1～2个月,记录其主要心血管不良事件。结果血清hs-TnT检测心肌坏死检出率为52.69%,高于常规TnT的检出率(32.26%)(P<0.05);血清hs-TnT水平随冠状动脉狭窄的严重程度及范围的扩大而升高(P<0.05),对冠状动脉病变程度及范围具有临床指导意义。随访1～2个月,hs-TnT升高组患者的主要心血管不良事件发生率高于hs-TnT正常组(P<0.05)。结论hs-TnT对急性冠状动脉综合征患者的临床诊断及短期预后价值优于常规TnT检测,对冠状动脉病变程度及范围具有临床指导意义,值得临床推广应用。

Serum high sensitivity of troponin T in prognostic value in patients with acute coronary syndrome

Objective To investigate the the clinical diagnosis and short-term prognostic value of serum high sensitivity troponin T(hs-TnT) in patients with acute coronary syndrome(ACS).Methods 186 cases of patients with acute coronary syndrome admitted to our hospital from January 2010 to June 2011 were selected and were given serum hs-TnT and conventional TnT testing within 24 hours after admission,coronary angiography was detected during hospitalization,hs-TnT and conventional TnT detection rate of myocardial necrosis was compared,hs-TnT levels and coronary artery disease extent and scope relationship were analyzed.All patients were followed up for 1 to 2 months,the major adverse cardiovascular events were recorded.Results The serum hs-TnT in detection of myocardial necrosis detection rate was 52.69%,which was higher than the conventional TnT detection rate of 32.26%(P < 0.05);The more serious coronary artery disease,the higher the level of serum hs-TnT(P < 0.05).All patients were followed up for 1 to 2 months,the major adverse cardiovascular events of hs-TnT high group were more than those of hs-TnT normal group(P < 0.05).Conclusion hs-TnT in the clinical diagnosis of acute coronary syndrome and short-term prognostic value is better than conventional TnT detection,it has clinical significance to coronary artery disease extent and scope,which is worthy of clinical application.
