乙型肝炎合并肝硬化术后并发症的危险因素

目的探讨影响乙型肝炎合并肝硬化术后并发症的主要危险因素。方法回顾性分析本院收治的乙型肝炎合并肝硬化患者186例,根据患者术后是否发生并发症将其分为病例组（32例）和对照组（154例）。病例组为术后发生并发症的患者,对照组为术后未发生并发症的患者。比较两组患者临床资料的差异对其术后发生并发症的影响。结果食管静脉曲张程度（OR=4.541,95%CI=1.128～8.293）、肝功能分级（OR=2.312,95%CI=1.214～5.637）、术中出血量（OR=1.928,95%CI=1.011～4.539）是患者术后发生并发症的危险因素。结论术前应对患者进行正确评估,分析围术期病理生理变化特点,以降低术后并发症的发生率及病死率。

The risk factors of postoperative complications of hepatitis B combined with cirrhosis

Objective To investigate the major risk factors of postoperative complications of hepatitis B combined with cirrhosis.Methods 186 patients with hepatitis B combined with cirrhosis were retrospectively analyzed,all patients were divided into case group and control group according to the exsitance of postoperative complications.Patients with postoperative complications as case group(32 cases),and patients without postoperative complications as control group(154 cases).The clinical data which effects of the postoperative complications of patients in two groups were compared.Results Esophageal varices(OR = 4.541,95%CI = 1.128~ 8.293),liver function grade(OR = 2.312,95%CI = 1.214~5.637),bleeding during operation(OR = 1.928,95%CI = 1.011~4.539) were risk factors of postoperative complications.Conclusion Patients should be correct assessmented in the preoperative.Analysis of pathophysiological changes of perioperative in order to reduce the incidence of postoperative complications and mortality rate.
