Wernicke脑病MRI特征及其诊断价值

目的探讨Wernicke脑病的MRI特征及其诊断价值。方法回顾性分析了11例Wernicke脑病患者的临床症状及其MR图像上特征性表现。结果11例Wernicke脑病患者其临床症状主要以眼外肌瘫痪、共济失调及意识障碍的三联征为主要特点。Wernicke脑病MRI特征主要表现为对称性长T1及长T2信号，于FLAIR序列中呈明显高信号，弥散加权成像（DWIb=1000）呈高信号；主要累及部位为乳头体，第三、四脑室；中脑导水管周围灰质。结论Wernicke脑病是由于维生素B．缺乏而引起的代谢性疾病的脑部表现；临床上有特定的病因；MRI表现具有一定的特征性．MRI对其病程和预后判断具有很大价值。

Features and the diagnostic value of MRI of Wernicke encephalopathy

Objective To explore MRI features of Wernicke encephalopathy and the diagnostic value of MRI.Methods The clinical symptoms and characteristic features on MR images date of 11 patients with Wernicke encephalopathy were analyzed retrospectively.Results 11 patients with Wernicke encephalopathy had various clinical symptoms and the extraocular muscle paralysis,ataxia and disturbance of consciousness of the triad was the main features.MRI characteristic images showed symmetrical long T1 and long T2 signal,intensity and hypertensity signal on FLAIR sequence as well as DWI imaging(DWIb=1000).Wernicke encephalopathy mainly involving parts of the nipple body,third,fourth ventricle,the periaqueductal gray.Conclusion Wernicke encephalopathy is the brain performance of metabolic diseases,which caused by lacking of vitamin B1.In the clinical Wernicke encephalopathy has a specific cause,In the MRI Wernicke encephalopathy shows a certain characteristic.So MRI has great value of judging the Wernicke encephalopathy course and the prognosis of Wernicke encephalopathy.
