耳部炎性疾病的细菌学及药物敏感性分析

摘    要:目的了解耳部炎性疾病的病原谱及药物抗菌谱规律,以指导临床合理用药,并作为临床经验用药的参考依据。方法选取2010年6月~2011年12月在我院就诊的128例慢性化脓性中耳炎及外耳道炎患者作为研究对象,所有研究对象均采取外耳道深部分泌物,进行pH测定、细菌及真菌培养和药物敏感性试验。结果耳部炎性疾病的主要病原菌是铜绿假单胞菌(30.36%)和金黄色葡萄球菌(25.89%),铜绿假单胞菌、大肠埃希菌、肺炎克雷伯菌以及鲍氏不动杆菌对头孢哌酮/舒巴坦、亚胺培南、哌拉西林/他唑巴坦耐药率较低,此类患者外耳道分泌物呈酸性。结论本研究结果可指导临床合理用药并可作为临床经验用药的参考依据,从而有助于减少该病的发生率,减少病程的迁延及避免严重并发症。

Abstract:Objective To explore the laws of antibacterial spectrum and etiologic spectrum in aural inflammatory diseases for guiding rational use of antibiotics and a reference in clinical experience medication.Methods A total of 128 patients with chronic suppurative otitis media of diffuse external otitis were enrolled into this study between June 2010 and December 2011.Deep external acoustic meatus purulence were collected from all study subjects.The pH value of deep external acoustic meatus purulence were determined.Deep external acoustic meatus purulence were cultured for bacteria,fungi and antimicrobial susceptibility test.Results The incidence rate of staphylococcus aureus and pseudomonas aeruginosa was 30.36% and 25.89% respectively.For pseudomonas aeruginosa,escherichia coli strains,klebsiella pneumoniae and acinetobacter baumannii,the antibiotics with the lowest resistance rate were cefoperazone/sulbactam,imipenem and piperacillin/sulbactam.Those patients discharge acid secretion.Conclusion Results of this study can paly a role in guiding rational use of antibiotics and can be regarded as a reference in clinical experience medication.Those are helpful to reduce the incidence rate of aural inflammatory diseases,shorten a long course of this disease and prevent serious complication.
