摘    要:目的:探讨术中胆道造影在小切口胆囊切除术中的应用价值。方法:共施行小切口胆囊切除术10020例,按相对胆管探查指征标准,施行1042例术中胆道造影:胆囊切除前经胆囊管造影、胆囊切除后经胆囊管造影及穿刺胆(肝)总管造影第三种。结果:发现病变119例,其中胆管结石113例,胆管变异4例,肝门部肿瘤2例。造影阳性率11.4%。结论:术中胆道造影能降低胆道阴性探查率、提供胆道解剖学依据、预防并及时发现胆道损伤,在小切口胆囊切除术中有重要作用。

Abstract:Objective To determine the role of intraoperative cholangiography (IOC) in surgery underwnt minilaparotomy cholecystectomy.Methods 10 020 cases of minilaparotomy cholecystectomy (MC) ,in which 1 042 cases underwent intraoperative cholangiography.There were three kinds of intraoperative cholangigorphy:through the cystic duct before remove of the gallbladder,through the cystic duct after remove of the gallbladder and through the common bile (hepatic )duct directly.Results Pathological changes of bile duct were found in 119 cases:113 cases with cholelith,4 cases with bile duct heteromorphosis and 2 cases with tumor in hepatichilum.The positive rate of IOC was 11.4%.Conclusion Intraoperative cholangiography can decrease the negative rate of common bile duct exploration,provide the bile duct anatomy,help prevent bile duct from damage and to find the damage early.In a world,it plays an important role in minilaparotomy cholecystectomy.
