儿童颅骨骨膜窦

目的：探讨儿童颅骨骨膜窦的诊断、治疗方法及疗效。方法：回顾分析我科1997～2002年收治的7例颅骨骨膜窦病例，其中男5例，女2例，年龄3个月～16岁，平均年龄4．5岁。病变位于颞部3例，额部2例，顶部1例，枕部1例。均行头颅CT和/或MRI检查，其中最近1例采用三维头颅CT扫描。手术方法采用切除颅外肿块，用骨蜡及生物蛋白胶封堵骨孔。结果：7例患儿术前均获影像学确诊，术后随访6个月～5年，无肿块复发。结论：CT、MRI能显示儿童颅骨骨膜窦的特性，三维头颅CT扫描更能清晰显示骨孔缺损部位，指导手术。对中小型且颅内无明显血管畸形病例，采用手术切除颅外肿块，骨蜡及生物蛋白胶封堵骨孔的方法，效果满意。

Diagnosis and treatment for pericranial sinus in children

Objective To study clinical features, diagnosis and treatment for pericranial sinus in children. Methods From July 1997 to Aug. 2002, 7 cases (male 5, female 2) with pericranial sinus were admitted. Their age ranged from 3 months to 16 years with an average of 4. 5 years. The lesions located in the temporal in 3 cases, the frontal in 2 cases, parietal one and occipital one. The diagnosis was confirmed by CT and/or MRI (n = 6), and three-dimensional CT (n = 1). The sinus was removed completely with the diploic blood channels of skull obliterated by bone wax and fibrin sealant. Results The follow-up out- come from 6 months to 5 years showed no relapse or any complications. Conclusions CT, MRI and 3-D CT are helpful in the diagnosis of pericranial sinus. Application of sinus removal with bone wax and fibrin sealant to closure diploic blood channels is suited for medium size of pericranial sinus but without vascular abnormal communications with the cerebral veins.
