Clinical analysis of 38 cases of gynecological malignancies with autoimmune diseases

【Abstract】?Objective?To investigate the clinicopathological features, treatment and prognosis of gynecological malignancies complicated with autoimmune diseases. Methods?A retrospective analysis was performed on 38 patients with gynecological malignancies complicated with rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), dermatomyositis (DM) and psoriasis who were hospitalized in the Department of Gynecology of Peking University People's Hospital from June 2004 to September 2019. Results?Among the 38 patients with gynecological tumor, 10 cases (10/38, 26.3%) were cervical cancer, 13 cases (13/38, 34.2%) were endometrial cancer, 15 cases (15/38, 39.5%) were ovarian cancer. 16 cases of FIGO stageⅠ(43.2%), 7 cases in stageⅡ(18.9%), 10 cases in stageⅢ(27.1%), and 4 cases in stageⅣ (10.8%).The median age of 38 patients was 54.5 years old.18 cases of them had RA (47.4%), 10 cases had SLE (26.3%), and 5 cases had DM (13.2%), 5 cases had psoriasis (13.2%).The gynecological malignancies complicated with RA, SLE and DM were mainly in patients with moderate to low differentiation. 29 patients who received adjuvant chemotherapy and /or radiotherapy after surgery hadⅡ~Ⅳdegree bone marrow suppression, 19 of them survived (73.1%), and 13 cases were alive more than 5 years. Conclusion?The prognosis of patients with gynecological malignancies complicated with autoimmune diseases is not only related to tumor stage, grade, pathological type and treatment, but also may be one of the factors affecting the prognosis of patients with gynecological malignancies. The mechanism may be related to tumor-related autoimmune antibodies.
