摘    要:丙泊酚是最常用的静脉麻醉药物之一,丙泊酚靶控输注(TCI)是结合丙泊酚药动学参数和预设血药浓度来计算输注速度,以达到血中稳定的丙泊酚浓度。但是丙泊酚的吸收、分布、代谢和排泄过程中存在显著个体差异,这成为影响TCI准确性的最重要因素。阐述了丙泊酚在体内过程研究的最新观点,结合三款具有代表性丙泊酚药动学模型,进一步揭示丙泊酚在体内的分布和代谢规律,指导临床医生针对不同临床情景制定丙泊酚的个体化应用策略。

Abstract:Propofol is one of the most commonly used intravenous anesthetics. Target-controlled infusion (TCI) is an infusion technique that combines the pharmacokinetic parameters of propofol and the expected blood concentration to calculate the infusion rate, in order to achieve a stable concentration of propofol in the blood. However, there are significant individual differences in the absorption, distribution, metabolism, and excretion of propofol between patients, which have become the most important factor affecting the accuracy of TCI. The latest viewpoints of propofol in vivo research are elaborated in this paper, and three representative models of propofol pharmacokinetics are combined to further reveal the distribution and metabolism of propofol in vivo, to guide clinicians to develop individualized application strategies of propofol for different clinical scenarios.
