不同绝经状态妇女绝经症状和性激素水平的对比研究

目的探讨不同绝经状态妇女的临床表现特点和血清性激素水平,为女性身心保健提供依据。方法将154例绝经状态的妇女按月经状态分成围绝经期组(n=48)、绝经早期组(n=65)和绝经晚期组(n=41),采用改良Kup-perman评分法对患者进行绝经症状评分,放免法检测血清雌二醇(E2)、促卵泡激素(FSH)、促黄体生成素(LH)水平,并进行比较分析。结果①Kupperman评分绝经早期组和绝经晚期组均显著高于围绝经期组(P<0.01);绝经早期组与绝经晚期组比较有统计学差异(P<0.05)。②3组改良Kupperman评分症状分级经χ2检验,差异有统计学意义P<0.05)。③绝经早期组和绝经晚期组血E2下降,FSH、LH升高,与围绝经期组比较,差异亦有统计学意义(P<0.05或P<0.01);绝经早期组与绝经晚期组比较均有统计学差异(P<0.01)。结论绝经期妇女改良Kupperman评分明显高于围绝经妇女,以绝经晚期更为显著,症状是否发生及其轻重程度与性激素(E2、FSH、LH)水平变化密切有关。

Comparative study of different with menopausal status and sex hormone level of women menopausal symptoms

Objective To compare the menopausal status and serum sex hormone levels of women＇s with menopausal symptoms, in order to provide important basis for women＇s physical and mental health. Methods 154 cases menopausal status women were divided into the perimenopausal group（n=48）, early postmenopausal group （n=65） and late menopause group （n=41） by menstrual state. Menopausal symptom score for patients using the modified Kupperman score, serum estradiol （E2） , follicle-stimulating hormone （FSH） , luteinizing hormone （LH） levels were measured by radioimmunoassay, and comparative analysis. Results （1） Kupperman score of earlypostmenopausal group and late menopause group were obviously higher than those in the perimenopausal group （P〈0. 01） ; the difference of early postmenopausal group and late menopause groups was statistically significant （P〈0. 05） ; （2） The modified Kupperman scoring symptomatic grading of three groups were statistically significant differences （P〈0.05） ;（3） compraing with perimenopa- usal group, blood E2 decreased, FSH, LH increased in early postmenopausal group and late menopause group, the differences were statistically significant （P〈0. 05 or P〈0.01）; the difference of early postmenopausal group and late menopause groups was statistically significant（P〈0. 01）. Conclusion The modified Kupperman score of postmenopausal women was significantly higher than menopausal women, and late menopause was more significant. The occurrence and severity of symptoms is closely related to sex hormones （E2, FSH, LH） level.
