经输尿管镜气压弹道碎石治疗尿路结石

目的总结经输尿管镜气压弹道碎石治疗尿路结石的治疗效果和经验。方法回顾性分析经输尿管镜气压弹道碎石术治疗尿路结石986例的临床资料。结果总体碎石成功率95％(937／986)，其中首次碎石成功率为89％(878／986)。术后主要并发症为血尿(35％，345／986)、腰痛(58％，572／986)。另有29例病人术后出现泌尿系感染，3例合并输尿管穿孔，17例出现输尿管狭窄，均经保守治疗或腔内手术治愈。结论经输尿管镜气压弹道碎石治疗尿路结石安全、高效、微创，是理想的治疗方法，而合理采用腔内治疗手段和熟练的操作技巧是提高手术成功率、减少并发症的关键。

Clinical Analysis of Ureteroscopic Lithotripsy for Urinary Calculi

Objective To evaluate the effect and the management experience of ureteroscopic pneumatic lithotripsy for urinary calculi. Methods 986 cases of urinary calculi who underwent ureteroscopic lithotripsy were retrospectively analyzed. Results The successful rate was 95%(937/986)and 89%(878/986)succeeded at the first time.The main postoperative complications were hematuria (35%,345/986)and lumbago (58%,572/986).Except these,there were also 29 cases of urinary infection,3 cases of ureteral perforation and 17 cases of ureterostenosis.All of them were cured with conservative treatment or endoscopic operations.Conclusion Ureteroscopic lithotripsy is an ideal method for urinary calculi and it is safe and effective with less injury.Reasonable adoption of this method and the improvement of operation skills are the key to increase the successful rate and decrease the complication rate.
