改良腹腔镜阑尾切除术80例

目的探讨腹腔镜下阑尾切除术（LA）阑尾系膜免钛夹处理的可行性。方法腹腔镜下从阑尾尖端开始紧贴阑尾壁电钩电凝切割游离阑尾膜的方法切除阑尾80例与传统腹腔镜阑尾切除术、开腹阑尾切除术的手术时间、术后疼痛、术后恢复时间及并发症进行比较。结果80例采用电钩分离法阑尾系膜游离的病例均获成功，无术后出血、肠瘘等并发症，手术时间、恢复时间、优于其它方法，术后疼痛、切口感染率住院时间等指标与传统腹腔镜阑尾切除术相似。结论电钩分离法阑尾系膜游离腹腔镜阑尾切除术安全可行，手术时间短，明显优于其它阑尾切除方法。

A modified laparoscopic appendectomy used in 80 cases

Objective To investigate the feasibility of dealing with the mesenterium of appendix without using titanium nip in laparoseopic appendeetomy（LA）. Methods Clinical data of 80 cases of LA using the modified method were assessed. The main point of the method was to use a eautery hook to eletrodisseet the membrane of appendix from head to root tightly close to the appendix wall. The result was compared with that of open appendectomy （OA） and conventional LA （CLA）. The postoperative pain, postoperative recovery time, and complications were compared. Results The modified laparoseopie appendectomy was successfully performed in all the 80 cases without complications such as postoperative bleeding and intestinal fistulas. The operative time and postoperative recovery time were shorter in LA than OA or CLA. Postoperative pains, incision infection and hospitalized time were similar to those of CLA. Conclusion The modified LA is a safe and feasible method. Its operative time and postoperative recovery time are short. The result confirms that it is obviously superior to other methods.
