磺(酰)脲类药物致严重低血糖20例临床分析

目的　为了了解老年Ⅱ型糖尿病患者服用磺 (酰 )脲类降糖药引起严重低血糖的情况。方法　总结了近 2年来 2 0例老年Ⅱ型糖尿病患者服用磺 (酰 )脲类降糖药引起严重低血糖的病例。结果　引起低血糖昏迷的病例 5例 ,他们分别服用格列本脲 (2例 )、格列齐特 (1例 )、格列吡嗪 (1例 )和格列喹酮 (1例 )。结论　应用此类药物的老年患者要注意年龄、肾功能状态和进食情况 ,同时注意向患者宣传糖尿病相关知识 ,预防严重低血糖的发生。

Clinic Analysis of 20 Cases with Severe Hypoglycemia Caused by Sulfonylureas

Objective to understand severe hypoglycemia during treatment ofsenior type Ⅱ diabtics with sulfonylureas. Method 20 cases of severe hypoglycemia in type Ⅱ diabtics with sulfonylureas were reported within 2 years. Result These patients, aged 60 or above, were treated with glibenclamide (15 cases), gliclazide (4 cases), glipizide(3 cases) and gliquidone(1 case)respectively. Severe hypoglycemic coma was seen in 2 cases with glibenclamide, 1 case with gliclazide, 1 case with glipizide and 1 case with gliquidone. Conclusion The reasons for hypoglycemia were due to aging, impairment of renal function, lack of caloric intake and overdose of sulfonylureas. We should pay more attention to their age, status of renal function and caloric intake when the aged diabetic patients take sulfonylureas, and propagate knowledge associated with diabetes mellitus to prevent the occurrence of severe hypoglycemia.
