全直肠系膜切除术治疗直肠癌105例临床分析

目的 探讨直肠系膜全切除（TME）在直肠癌保肛手术中的价值。方法 对105例不同分期的直肠癌患者采用TME原则的前切除术后局部复发率与对照组进行分析。结果 TME组的术后局部复发率6．7％，其中B期3．9％，C期11．1％，非TME组的局部复发率18．9％，其中B期12．5％，C期32．5％。结论 TME原则可有效地降低直肠癌保肛术后的局部复发率。Dukes分期是影响术后局部复发的一个重要因素，早期诊治是降低术后局部复发的重要环节。

The rectal cancer with TME operation:105 cases clinical analysis

Objective To evaluate the val ue of total mesorector excision(TME) for rectal carcinoma operation with preservat ive of anus.Methods 105 cases of different stages rectal cancer patients using TME principle were studied and compared with control groups in p ostoperative recurrence rate.Results Local recurrence rate in T ME postoperative groups was 6.7%,among them stage B 3.9%,stage C 11.1%.Postopera tive recurrence rate in non-TME groups was 18.9%(stage B 12.5%,stage C 32.5%). Conclusion TME principle can effectively lower local recurrence rate in rectal cancer with preservation of anus.Dukes staging is an important f actor influencing local recurrence.Early diagnosis and treatment play a key role in lowering postoperative local recurrence.
