宫腔镜及三维超声造影对子宫内膜器质性病变的诊断价值

摘    要:目的评价宫腔镜及三维超声宫腔造影检查方法对子宫内膜器质性病变的诊断价值.方法回顾性分析54例子宫不正常出血的病人,应用经阴道三维超声宫腔内注液后扫查,再行宫腔镜检查,以病理结果为确诊依据对照不同检查结果间的差异.结果 23例诊断为子宫内膜息肉,15例诊断为黏膜下肌瘤,6例诊断为子宫内膜癌,10例诊断为子宫内膜增殖症.宫腔镜检查敏感度100%,特异度98.1%.三维超声造影检查敏感度为100%,特异度94.4%.结论宫腔镜为诊断子宫内膜占位病变的首选方法,结合三维超声宫腔造影,对确定治疗方案有很大辅助作用.

Abstract:Objective: To evaluate the diagnostic value of hysteroscopy combining three-dimensional ultrasonic hysterography on the endometrial organic changes.Methods: Fifity-four women who had been suffered from irregular uterine bleeding were collected. After intrauterine injection by trans-vaginal ultrasound, we adopted three-dimensional ultrasound scanning, combined hysteroscopy examination. We contrasted the diagnosis differences between various methods, taking pathologic results as the definite diagnosis. Results:Twenty-three patients were diagnosed endometrial polyp, 16 patients were submucosal myoma, 6 patients were endometrial carcinoma, 10 patients were endometrial hyperplasia. The sensitivity of hysteroscopy exam is 100%, the specificity is 98.1%.The sensitivity of three-dimensional ultrasonic hysterography is 100%,the specificity is 94.4%.Conclusions:Hysteroscopy and three-dimensional ultrasonic hysterography are complementary methods, combining them can diagnose endometrial organic changes much precisely, and direct clinical therapy meanwhile.
