血清β_2微球蛋白与银屑病

目的:为明确银屑病与β_2微球蛋白(β_2-M)之间的关系。方法:对确诊的80例寻常型银屑病患者进行血清β_2-M水平的检测,同时测定了血清免疫球蛋白的浓度。β_2-M的检测采用125放射免疫分析法。结果:寻常型银屑病血清β_2-M水平显著性增高。尤以进展期为甚(与对照组比较,P值均<0.01),进展期升高又较稳定期明显(P<0.01)。地图状较点滴状高(P<0.01)。寻常型银屑病血清免疫球蛋白IgA浓度显著高于对照组(P<0.01)。而IgA浓度则显著低于对照组(P<0.05)。结论:从检测结果分析,寻常型银屑病患者血清中存在的自身抗体,可能包括角朊细胞,淋巴细胞等在内的有核细胞过多地破坏,使得血清β_2-M浓度显著升高,尤以进展期为甚。寻常型稳定期银屑病患者体内可能也存在一定量自身抗体。使得血清β_2-M浓度升高,一定程度上解释了临床上银屑病稳定期和进展期反复交替的现象,以及治愈的银屑病复发率比正常的人群发病率高的可能原因。寻常银屑病患者血清免疫球蛋白的变化也进一步说明寻常型银屑病存在体液免疫功能紊乱,检测的结果从一个侧面支持了寻常型银屑病并发病与自身免疫有关。

Psoriasis and Serum β2-microglobulin

Objective: To study the relationship between psoriasis and β2-microglobulin. Methods: The serum levels of β2-mi-croglobulin were determined by radioimmunoassay in 80 patients with vulgar psoriasis. Serum levels of immunoglobulin were also tested. Reasults: The serum level of β2-microglobulin was significantly higher in the psoriasis patients than in control group.The level was higher in the progressive stage than in the stable stage;it was higher in geographic.The serum level of IgA was significantly higher in the psoriasis patients than in control group.The serum level of IgM was lower in the psoriasis patients than in control group. Conclusion: there were autoantibodies to keratinocyte and lymphocyte in the serum of psoriasis vulgar patients.The destruction of these cells led to more β2-M in the serum, especially in progressive stage. There are some autoantibodies in the serum of patients with vulgar psoriasis, in stable stage, too, which led to more β2-M in the serum, and the increase of β2-M accounts for the alteration of stable stage and progressive stage in patients with psoriasis in some degree, and the reasan of the higher ratio of psoriasis in the cured psoriasis patients than healthy man. The abnormality of serum immunoglobulin also suggests that there are disturbance of humoral immunity in psoriasis and that vulgar psoriasis is related to human autoimmunizatian in some way.
