血管内栓塞与手术切除联合治疗巨大脑动静脉畸形

为探讨术前血管内栓塞在治疗巨大脑动静脉畸形（ＡＶＭｓ）中的作用，作者从１９９６年８月至１９９８年４月对２７例巨大ＡＶＭｓ的患者进行手术治疗。每个患者于术前均行ＡＶＭｓ血管内栓塞。结果显示先行血管内栓塞后进行手术切除可明显减少术中出血，大大降低术后致残率和死亡率，并可有效地防止正常灌注压突破综合征（ＮＰＰＢ）。本研究提示血管内栓塞与手术切除联合治疗巨大脑ＡＶＭ是一种行之有效的手段。

Combination of surgical resection with intravascular embolization for treatment of giant cerebral arteriovenous malformations

To investigate the effects of preoperative intravascular embolization on treatment of intracranial giant arteriovenous malformations(AVMs), we have successfully operations on 27 patients with intracranial giant AVMs from Augest, 1997 to April, 1998 Every patient was treated with preoperative intravascular embolization Compared with surgical resection alone, we found that the surgical resection after intravascular embolization can reduce the intraoperative hemorrhage greatly, diminish the postoperative mortality or morbidity and prevent normal perfusion pressure breakthrough syndrome(NPPB) significantly The present study suggest that combination of surgical resection with intravascular embolization is an effective way in the treatment of giant cerebral AVMs
