肝硬化患者血清肝功能指标与肝纤维化标志物的相关性研究

目的探讨肝血清纤维化指标与肝功能指标的相关性。方法按Child-Pugh分级标准将100例肝硬化患者分为A、B、C三级组,选择30例健康体检者作为正常对照组。测定肝纤维化4项指标和肝功能4项指标。结果结果在A、B、C三级组中,TBIL、TBA水平随着Child-Pugh积分的增高而升高(P<0.05);CHE和ALB水平随着Child-Pugh积分的增高而下降(P<0.05);血清肝纤维化标志物水平随Child-Pugh积分增加而呈上升趋势(P<0.05),肝纤维化4项指标与肝功能4项指标之间有相关性(r=0.8580～0.0053,P<0.05),与TBIL、TBA呈正相关,与CHE、ALB呈负相关。结论联合检测血清CHE、ALB、TBIL、TBA及PCⅢ、Ⅳ-C、LN、HA水平对估测肝硬化的严重程度及预后有十分重要的临床价值。

Relationship of Liver Function Indexes with the Liver Fibrosis Indexes in Patients with Cirrhosis

Objective To investigate the relationship of liver function indexes with the liver fibrosis indexes in patients with cirrhosis. Methods 100 patients with cirrhosis of liver were divided into stage A, B and C according to Child-Pugh staging. 30 cases of healthy individuals were as control group. The liver fibrosis indexes and the liver function indexes were measured. Results TBIL and TBA level with Child-Pugh score increased and raised, A-grade, B-grade and C-grade compared the difference was significant （P〈 0.05）. CHE and ALB level with Child-Pugh score increased and decreased, A-grade, B-grade and C-grade compared the difference was significant （P〈 0.05）. PC III, IV-C, LN and HA level with Child-Pugh score increased and raised, A-grade, B-grade and C-grade compared the difference was significant （P〈0.05）. The liver function indexes with the liver fibrosis indexes have relevance （r=0.8580-0.0053, P〈 0.05）, with TBIL, TBA was positively correlated and with the CHE, ALB was a negative correlation. Conclusion Combined measuring the liver fibrosis indexes and liver function indexes could as a judge for predicting the degrees of liver cirrhosis and prognosis of liver cirrhosis.
