肺癌术后并发症心肺危险指数以外的影响因素分析

目的进一步探讨心肺危险指数（CPRI）以外还有哪些因素与肺癌术后并发症的发生有关。方法统计医院从1997年3月至2007年3月间接受肺切除手术治疗的129例患者资料，分析并发症情况及其与患者性别、TNM分期、手术时间、手术涉及肺脏范围、影像学特征、术前治疗之间的相关性。结果129例患者中49例术后发生并发症，CPRI能较好预测肺癌术后近期并发症。除此以外，肺癌TNM分期高、手术时间长、手术范围广，术后并发症发生率就高（P〈0．05）；男性患者组、中央型肺癌组、术前未预防性治疗组比各自对应组术后并发症发生率高，但差异未见统计学意义（P〉0．05）。结论CPRI以外还有一些因素影响着肺癌术后并发症的发生，可以作为预测术后并发症的参考指标。

Postoperative Complications in Lung Cancer Analysis of Affecting Factors Besides Cardiopulmonary Risk Index

Objective To profoundly investigate other factors,out of cardiopulmonary risk index ( CPRI) ,of postoperative complications in lung cancer.Methods To study the possible factors (namely gender,TNM staging,operation duration,operation scope,image characteristics and preoperative treatment) and postoperative complications in 129 lung cancer patients undergone surgical treatment from March 1997 to March 2007 in the hospital.Results A total of 129 consecutive patients were studied and postoperative complications occurred in 49 patients.CPRI showed significant importance in predicting the early complications for patients with NSCL.Meanwhile,the higher incidence of postoperative complications was caused by the higher TNM staging,the longer operation duration and the broader operation scope (P < 0.05).The incidence was higher in the male patients,center type lung cancer and those without preoperative treatment,however the difference was not significant. Conclusion Besides CPRI,there are other factors which affect the incidence of postoperative complications,they can be used as the additional index for predicting the incidence of postoperative complications in lung cancer.
