帕瑞昔布术前镇痛预防全麻拔管期应激反应

目的：探讨帕瑞昔布钠预防全麻拔管期应激的有效性和安全性.方法：全麻下行妇科腹腔镜手术100例患者分为观察组和对照组.观察组在术前静滴帕瑞昔布钠40 mg,对照组不作任何处理.观察并记录入室后各时段的MAP、HR变化.评价患者拔管期合作情况.记录麻醉苏醒时间、喉痉挛发生率、呛咳发生率、恶心呕吐发生率.结果：观察组患者在拔管即刻（T2）,拔管后5 min（T3）,10 min（T4）的MAP、HR变化与术前（TO）相比,差异无统计学意义.对照组在上述各时点的MAP、HR与观察组相比,差异有统计学意义（P〈0.05）.两组在拔管期合作情况相比,差异有统计学意义（P〈0.05）,其他指标差异无统计学意义.结论：术前即用帕瑞昔布镇痛,效果好,不良反应少,且患者合作程度高,是预防全麻拔管期应激反应的较好药物,值得推广.

Prevention Effects of Parecoxib Preemptive Analgesia on Stress Reaction of Extubation after General Anesthesia

Objective： To observe the validity and safety of parecoxib injection to prevent the stress reaction of extubation after general anesthesia. Method： One hundred patients received gynecological laparoscopy under general anesthesia were randomly divided into control group （n = 50） and observation group （n = 50）. The patients in observation group were ivd parecoxib（40 mg） before anesthesia. The different period of two groups with MAP and HR were observed. The cooperation of patients was evaluated in extubation. The recovery time, the incidence of laryngospasm, cough, nausea and vomiting of patients were recorded and observed. Result： Campare with T2,T3 and T4,the MAP and HR of observation group in To were no statistical difference. But the MAP and HR were obviously increased in control group （P 〈 0. 05）. There were statistical difference in the recovery time （ P 〈 0. 05 ）. There were no statistical difference in other indexes. Conclusion： Parecoxib injection before operation has preemptive analgesic effect in inhibiting the stress reaction of extubation after general anesthesia. It is an effective method with low side effects for the patients under general anesthesia.
