婴儿特发性血小板减少性紫癜134例临床分析

目的探讨婴儿特发性血小板减少性紫癜（ITP）的临床特点。方法对我院收治的134例ITP婴儿的临床资料进行分析。结果134例ITP婴儿以男婴为主，男：女=3．96：1；年龄以6个月以下多见（74．6％）。临床多表现为发病急，出血症状轻，但血小板数量重度减少多见（69．4％）。发病前有感染史57例（42．5％）。血小板抗体检查阳性率52．7％。巨细胞病毒的感染率（51．7％）明显高于EB病毒和肺炎支原体的感染率。肾上腺皮质激素联合人免疫球蛋白治疗93例，有效率达88．2％。结论婴儿ITP大多发病急，血小板数量重度减少，多数有病毒感染，肾上腺皮质激素联用人免疫球蛋白治疗效果明显。

Clinical analysis of 134 cases with idiopathic thrombocytopenic purpura in infants

Objective To discuss the clinical characteristics of idiopathic thrombocytopenic purpura （ITP） in infants. Methods The clinical data of 134 cases of ITP were analyzed. Results The male vs. female was 3.96 ： 1 in 134 infants with ITP. The age was below six months in 74.6% of the patients. Most of clinical manifestations were acute type and slight bleeding, and platelet count decreased significantly. 42.5% of them were infected, and the positive rate of platelet antibody was 52.7% . Viral infection were associated with ITP in infants. Cytomegalovirus （CMV） infection rate （51.7%） was higher than that of epstein barr virus （EBV） and Mycoplasma pneumoniae. Effective rate was 88.2% of 93 cases after the therapy with cortieoid plus human immunoglohulin. Conclusion Most of ITP in infants are acute type; Platelet count decreases significantly; Viral infection is associated with the disease; The therapy with corticoid plus human immunoglobulin has a good effect.
