亚低温对重型颅脑损伤患者术后颅内压及预后的影响

摘    要:目的 了解亚低温对重型颅脑损伤患者开颅血肿清除及减压术后颅内压、脑灌注压及预后的影响。方法 通过对 6 1例重型颅脑损伤患者在急诊开颅血肿清除及减压术后 ,随机分为亚低温组及常温组进行治疗 ,治疗过程中对两组患者的颅内压及脑灌注压进行持续监测 ,并分别取术后开始治疗时和术后第 3、第 7天的颅内压 ,脑灌注压及预后情况进行分析。结果 亚低温组患者术后第 3天及第 7天的颅内压低于常温组 ,脑灌注压高于常温组。根据GOS评分 ,亚低温组患者的病死率及恢复良好率均优于常温组。结论 低温治疗可有效降低重型颅脑损伤患者术后的颅内压 ,提高患者的脑灌注压 ,改善患者的预后。

Abstract:Objective To study the effect of mild hypothermia on intracranial pressure(ICP)and cerebral perfusion pressure(CPP) and prognosis of patients with severe head injury after evacuation and decompression of intracranial hematoma.Methods 61 cases with severe head injury after evacuation and decompression of intracranial hematoma were randomly divided into the hypothermia group and the control group.ICP and CPP were continuously monitored during the therapeutic process.The ICP and the CPP before and 3,7 days after the therapy,as well as the prognosis,were statistically analyzed respectively.Results On the 3rd and 7th day postoperative,the ICP of hypothermic group was lower than that of control group,and the CPP of hypothermia was higher than that of control group.According to Glasgow outcome Scale (GOS),the mortality rate was lower and the recovery rate was higher in the hypothermic group than that in the control group.Conclusions Mild hypothermia can decrease ICP and increase the CPP of patients with severe head injury after surgery for intracranial hematoma,and improve the prognosis.
