卡介苗膀胱灌注防治上尿路移行细胞癌术后复发

目的 :观察卡介苗 (BCG)化学免疫预防上尿路移行细胞癌术后复发的疗效。方法 :对 50例肾盂及输尿管癌患者术后应用BCG 12 0mg与常温盐水 60mL膀胱灌注 ,每周 1次 ,共 6次 ;每 2周 1次共6次 ;每 4周 1次至 2年 ;每 3个月 1次维持 5年。结果 :随访 1～ 15年 ,平均 8年 ,1、3、5年生存率分别为 92 0 % ( 4 6/ 50 )、77 8% ( 31/ 4 0 )、59 4 % ( 19/ 32 ) ;1、3、5年局部复发率分别为 2 0 % ( 1/ 50 )、15 0 %( 6/ 4 0 )、2 5 0 % ( 8/ 32 )。结论 :BCG膀胱灌注可减少上尿路移行细胞癌局部复发 ,提高 5年生存率。

Prevention and Cure of Transitional Cell Carcinoma in the Upper Ureter Using BCG Perfusion into the Urinary Bladder

Objective To observe the effect of BCG in preventing the recurrence of transitional cell carcinoma immunochemically.Methods Fifty cases of postoperative pyelo ureter carcinoma were treated by BCG perfusion into the urinary bladder at a dose of 120 mg in 60 ml saline at room temperature.The BCG perfusion was carried out once a week,6 times in all;once every 2 weeks,6 times in all;once every 4 weeks,lasted for 2 years;once every 3 months,lasted for 5 years,consecutively.Results In 1-15 years of follow up (8 years in average),the livability in 1,3,5 year was 92%(46/50),80%(40/50),64%(18/50) respectively and the local recurrent rate in 1,3,5 year was 2.1%(1/46),13%(6/46),21.7%(10/46) respectively.Conclusion BCG perfusion into the urinary bladder can decrease the local recurrence of transitional cell carcinoma in upper ureter,and can enhance the five year livability.
