球形肺炎的CT诊断

目的 探讨球形肺炎的cT征象，提高对本病的认识。方法 回顾性分析经临床证实的26例球形肺炎的CT资料。结果 病灶部位：两肺下叶背段及基底段18例，上叶后及尖后段5例，中叶内侧及上叶下舌段3例。主要CT表现：病灶呈方形、三角形18例，类圆形8例，均与胸膜接触；周边密度较低20例，其中呈晕圈征9例；边缘有模糊的粗、长毛刺或锯齿征19例；空气支气管征5例；病灶与肺门间有增粗的血管纹理相连21例；病变非胸膜侧见小片状模糊影16例；邻近胸膜广泛均匀性增厚26例；抗炎治疗后复查病灶吸收缩小26例。结论 病灶部位与CT征象对球型肺炎的诊断、鉴别诊断有很大的价值。

CT diagnosis of spherical pneumonia

Objective To investigate the CT appearance of spherica l pneumonia so as to elevate the recognition of signs suggestive of this diagnosis .Methods The CT findings of 26 patients with clinically proven spherical pneumon ia were analyzed retrospectively for location,pattern and secondary signs sugges ting the diagnosis.Results For the distribution of the lesions,the back and basa l segments of the both lower lobes were found in 18 cases,posterior and tip po sterior segments of the upper lobe in 5 ,and inner side of the middle lobe and l ower lingual segments of the upper lobe in 3.For the pattern of the lesions,the square or triangular lesions were found in 18 cases,round- like lesions in 8.Al l lesions were contacted widely with pleurae.The peripheral density of the lesio ns in 20 cases was lower than that of central area,of them,9 cases had halo sign s.The coarse and long spicalations or serrated margins were found in 19 cases,ai r bronchogram in 5,vessel convergence between the lesion and hilum of lung in 21 ,small patchy hazy shadows on non- pleural side of the lesion in 16,and extensi vely pleural thickening in 26.Following anti inflammatory therapy for short te rm the all lesions obviously reduced or even disappeared.Conclusion The location s and CT signs of the lesions have great value in the diagnosis and differential diagnosis of spherical pneumonia.
