经额底纵裂入路显微手术切除巨大垂体腺瘤

目的探讨经额底纵裂入路显微手术切除巨大垂体腺瘤的优缺点。方法回顾性分析了15例经额底纵裂入路显微手术切除巨大垂体腺瘤的临床资料。结果本组患者肿瘤全切除11例,次全切除4例,无手术死亡,全切除率达73.3%。大部分患者术前症状明显改善,无严重术后并发症。结论经额底纵裂入路显微手术切除巨大垂体腺瘤,为术者提供了安全广阔的手术视野,可直视下分离切除肿瘤,易于保护垂体柄,对下丘脑、颈内动脉的损伤亦较轻微,术后并发症少。

Microsurgical Resection of Huge Pituitary Adenoma Via Subfrontal combined Longitudinal Fission Approach

Objective To study the advantages and disadvantages of microsurgical resection of huge pituitary adenoma via subfrontal combined longitudinal fission approach. Method Retrospective analysis of 15 cases of huge pituitary adenoma treated by the approach of subfrontal combined longitudinal fi ssion. Results Eleven cases of huge pituitary adenoma were totally resected,4 of huge pituitary adenoma underwent subtotal resection,no patient died post-operatively. The totally resection rates reached 73.3%,and the clinical symptoms were signifi cantly relieved and no severe complications were found. Conclusions Huge pituitary adenoma by subfrontal combined longitudinal f ission approach was resected in a better f ield and the pituitary stalk was protected in a better condition,so that slightly damage of hypothalamus and internal carotid artery and few complications were obtained.
