舌鳞状细胞癌预后多因素分析

目的探讨影响舌鳞状细胞癌预后的相关临床等因素。方法回顾性分析175例舌鳞状细胞癌的长期随访资料,采用SPSS 13.0统计软件进行分析。频数资料组间比较采用χ2检验。临床病理指标单因素分析采用Kaplan-Meier法,组间曲线比较用Log-Rank检验,多因素分析采用Cox回归分析。结果全组患者3、5年生存率分别为76.5%、65.4%。单因素分析显示,肿瘤大小、肿瘤是否越过中线、舌外是否侵犯、组织学分级、临床分期、淋巴结转移、局部复发、治疗方式是影响预后的重要因素(P<0.05);多因素分析显示,临床分期、组织学分级、局部复发是影响舌鳞状细胞癌预后的重要因素(P<0.05)。结论影响舌鳞状细胞癌预后的主要因素是临床分期、组织学分级、局部复发。

Multivariate regression analysis on prognosis of tongue squamous cell carcinoma

Objective To investigate the clinical factors related to the prognosis of tongue squamous cell carcinoma.Methods The clinical information and follow-up data of 175 patients with tongue squamous cell carcinoma were analyzed retrospectively.SPSS13.0 software was used for statistic treatment.Chi-square test was used for the comparison of frequency data between groups.Kaplan-Meier method was applied to analyze survivals.Log-Rank was used to compare curves between groups.Cox regression was applied for multivariate prognostic analysis.Statistical significance was considered at P0.05.Results The 3-year,5-year survival rates were 76.5% and 65.4%,respectively.Univariate analysis showed that tumor size,extent of tumor invasion,histological grade,clinical stage,local recurrence,lymph node metastases and treatment modality were associated with prognosis.Multivariate analysis showed that histological grade,clinical stage and local recurrence were prognostic factors for patients with tongue squamous carcinoma.Conclusion Histological grade,clinical stage and local recurrence are important prognostic factors in human tongue squamous cell carcinoma.
