可吸收骨固定系统治疗颌面部骨折的临床研究

摘    要:目的 :探讨可吸收骨固定系统在治疗颌面部骨折中的作用。方法 :36例各种原因造成的颌骨骨折的患者,由患者及家属自主选择内固定材料,分为研究组19例和对照组17例。研究组,通过可吸收骨固定系统治疗颌面部骨折;对照组,通过金属骨固定系统治疗颌面部骨折。术后6、12、18、24周进行定期检查,随访创口愈合情况,影像学复查,进行临床综合疗效评估。结果:术后12周研究组显效16例、有效3例;对照组显效8例、有效7例,进步2例。两组患者术后临床疗效差异有统计学意义(P<0.05)。术后24周,研究组显效17例,有效2例;对照组显效15例,有效2例,两组患者术后临床疗效差异无统计学意义(P>0.05)。两组患者24周研究组骨密度优良率高于对照组(P<0.05)。结论:在正确掌握适应证,选择合适病例的前提下,利用可吸收骨固定系统治疗颌骨骨折的患者是一种较理想的治疗方法。临床疗效有待进一步严格随机分组的临床研究证实。

Application of Absorbable Bone Fixation System in the Treatment of Maxillofacial Fractures
Abstract:Objective: To investigate the efficacy of absorbable maxillofacial bone fixation system in the treatment of fractures. Methods: 36 patients suffered from maxillofacial fractures were randomly divided into observation group (19 cases) and control group (17 cases). In the observation group, through the use of absorbable bone fixation system in the treatment of maxillofacial fractures;control group using a metal bone fixation systems for reduction. The patients were re-examined regularly at 6 weeks, 12 weeks, 18 weeks, and 24 weeks respectively. The wound healing status, radiolographic examinations, and comprehensive clinical efficacy evaluation were underwent. Results: At 12 weeks follow-up in the ob-servation group, 16 cases displayed markedly effective, and 3 cases showed moderate effective; while in the control group, 8 cases displayed markedly effective, 7 cases showed moderate effective, 2 cases showed slight effective. Clinical efficacy differences between two groups was statistically significant (P<0.05). At 24 weeks follow-up, 17 cases displayed markedly effective, 2 cases moderate effective in the observation group; while in control group, 15 cases displayed markedly effec-tive, 2 cases displayed moderate effective, with no differences between groups (P>0.05). Comparing with bone mineral density (BMD) excellent rate at 24 weeks, observation group was higher than that of the conventional titanium plate group (P<0.05). Conclusion:Absorbable bone fixation system may be an alternative way in the treatment of maxillofacial fractures.
