单纯化疗治疗尤文肉瘤术后脑、肺转移的临床效果分析

摘    要:目的探讨单纯化疗对尤文肉瘤术后脑、肺多发转移的治疗效果。方法回顾分析2013年1月收治的1例尤文肉瘤术后患者的临床资料。男性,25岁。2011年4月于我院确诊为左股骨上段尤文肉瘤,按VAC-IE方案化疗1个疗程后,行瘤段切除,肿瘤假体置换。术后病人未按医嘱要求进行化疗,2a后出现脑、肺转移。再次按照VAC+IE方案化疗。结果再次化疗1个疗程后,病人头痛症状明显缓解。肺部X线片示治疗后肺野内占位消失。头颅CT平扫示化疗后脑实质转移灶较化疗前显著缩小,脑室系统对称。结论VAC-IE化疗方案对尤文肉瘤脑、肺多发转移的治疗有效。

Chemotherapy as an effective treatment for multiple metastases of lung and brain after surgery in a case of ewing sarcoma

Abstract:Objective The treatment for multiple metastases of Ewing sarcoma is rarely reported. The pa- per is to study the curative effect of chemotherapy for multiple metastases of lung and brain after surgery in a case of Ewing sarcoma. Method A 25-year-old male was admitted to hospital in January 2013, and the clinical data were analysed. In April 2011, he was diagnosed with Ewing sarcoma of the left proximal fe- mur. The treatment consisted of enbolc resetion with femoral prosthesis replacement, completed by chem- otherapy (VAC+IE regimen) before surgery. However, chemotherapy was not performed as required af- ter surgery, multiple metastases of lung and brain occurred two years later. Chemotherapy (VAC+IE reg- imen) was performed again. Results The symptom of headache was observably relieved after chemothera- py. Chest X-ray before and after the chemotherapy showed the space-occupying lesions disappeared. Head CT scan before and after the chemotherapy showed shrinkage of metastases with symmetrical ventricle. Conclusion VAC+IE as chemotherapy regimen of Ewing sarcoma is an effective treatment for multiple metastases of lung and brain.
