筛骨纸样板爆裂骨折的CT诊断

摘    要:目的 探讨CT对筛骨纸样板爆裂骨折的诊断价值。方法 回顾性分析临床及影像资料完整的41例筛骨纸样板爆裂骨折的部位、分型及周围组织结构情况。结果 右侧纸样板爆裂骨折15例(36.6%,15/41),左侧19例(46.3%,19/41),双侧7例(17.1%,7/41)。纸样板线型骨折9例(22.0%,9/41),凹陷性骨折32例(78.0%,32/41)。纸样板骨折伴内直肌肿胀、迂曲30例,内直肌及眶内脂肪疝入筛窦32例,内直肌与纸样板间脂肪间隙变窄或消失34例,眶内积气8例,眼球内陷4例,眼球突出1例,视神经肿胀3例,筛窦塌陷窦腔变小32例,筛窦外伤性积液20例。结论 CT不仅能准确地显示筛骨纸样板爆裂骨折的直接和间接征像,而且能很好地对其进行分型,CT对纸样板爆裂骨折的诊断具有重要价值。

CT Diagnosis of Lamina Papyracea Blowout Fracture

Abstract:Objective To study the diagnostic value of CT about lamina papyracea blowout fracture. Methods 41 cases were analysed retrospectively. Orbit CT scanning were performed in all cases. Results Right lateral in 15 cases (36.66%,15/ 41), left lateral in 19 cases (46.3%,19/41), both laterals in 7 cases (17.1%,7/41), fracture lines in 9 cases (22.0%,9/41), depression in 32 cases (78.0%,32/41), medio-rectus muscle hypertrophy in 30 cases, hernia of ocular muscle and orbital fat into ethmoid sinuses in 32 cases, narrowed or disappeared space between medio-rectus muscle and lamina papyracea in 34 cases, pneumatosis of orbit in 8 cases, eyeball concave in 4 cases, eyeball convex in 1 cases, optic nerve hypertrophy in 3 cases, collapse of ethmoid sinus in 32 cases, liquid accumulation in ethmoid sinus in 20 cases. Conclusion CT scanning ,not only accurately displayed the direct and indirect signs but also displayed the shape of lamina papyracea blowout fracture, is a valuable method in showing lamina papyracea blowout fracture.
