摘    要:目的]分析呼吸机相关性肺炎(VAP)的影响因素,为制定相应的预防措施提供依据。方法]采用 Logistic 回归分析法对某三级甲等医院中心 ICU 2011年1月-2013年6月发生的53例 VAP病人进行相关影响因素分析。结果] VAP发生率12.7%,病原菌感染以革兰阴性菌为主,Logistic 回归分析显示,基础疾病个数(OR=7.22,P〈0.01)、机械通气时间(OR=10.37,P〈0.001)、声门下吸引(OR=0.01,P〈0.001)、误吸(OR=8.36, P〈0.05)为 VAP 相关因素。结论]对ICU病人中存在 VAP 危险因素的高危人群,应加强 VAP监测与控制,并制定适当的措施来预防其发生。

Logistic regression analysis on influencing factors of ventilator associated pneumonia

Abstract:Obj ective:To analyze the influencing factors of ventilator associated pneumonia (VAP ),so as to provide evidences for making corresponding preventions.Methods:Logistic regression was used for analysis on the related influencing factors of 5 3 VAP patients in ICU in a three grade class A hospital from Jan 2011 to Jun 2013.Results:The incidence of VAP was 1 2 .7%.Gram negative bacterias were the main pathogen in infection.Logistic regression analysis showed that the VAP related factors included the number of basic disease (OR=7.22, P〈0.01),duration of MV (OR=10.37,P〈0.001),continuous aspiration of subglottic secretion (OR=0.01,P〈0.001)and aspiration (OR=8.36,P〈0.05).Conclusion:For the high risk groups with VAP risk factors in ICU patients,the monitoring and controlling of VAP should be enhanced.Some appropriate measures should be taken to prevent the occurrence of VAP.
