头部电针透穴、体针联合氟西汀治疗脑卒中后抑郁临床研究

目的:观察头部电针透穴、体针联合氟西汀治疗对脑卒中后抑郁患者脑电图及抑郁情况的影响。方法:选取122例脑卒中后抑郁患者为研究对象,按随机数字表法分为对照组和研究组各61例。对照组给予盐酸氟西汀胶囊治疗,研究组在此基础上联合头部电针透穴、体针治疗,2组均治疗4周。对比2组临床疗效,评估患者治疗前后的脑电图及抑郁情况。结果:研究组总有效率86.89%,高于对照组的68.85%,差异有统计学意义(P<0.05)。与治疗前比较,治疗后2组脑电图正常患者比率均升高,研究组脑电图正常患者比率高于对照组,差异均有统计学意义(P<0.05)。与治疗前比较,治疗后2组汉密尔顿抑郁量表(HAMD)评分均降低,且研究组HAMD评分低于对照组,差异均有统计学意义(P<0.05);2组血浆5-羟吲哚乙酸(5-HIAA)水平均升高,且研究组血浆5-HIAA水平高于对照组,差异均有统计学意义(P<0.05)。结论:头部电针透穴、体针联合氟西汀可有效改善脑卒中后抑郁患者的临床症状,调节5-HIAA水平,疗效优于单纯使用氟西汀治疗。

Clinical Study of Scalp Point-through-point Electroacupuncture and Body Acupuncture Combined with Fluoxetine for Post-stroke Depression

Objective: To observe the effect of scalp point-through-point electroacupuncture and body acupuncture combined with fluoxetine on electroencephalogram and depression in patients with post-stroke depression. Methods:A total of 122 cases of patients with post-stroke depression were selected as the study subjects,and were divided into the control group and the study group according to the random number table method,61 cases in each group respectively. The control group was treated with fluoxetine hydrochloride capsules, and the study group was additionally treated with scalp pointthrough-point electroacupuncture and body acupuncture based on the treatment of the control group. Both groups were treated for four weeks. The clinical effect in the two groups was compared and analyzed. Before and after treatment, the electroencephalogram and depression were evaluated. Results: The total effective rate was 86.89% in the study group,higher than that of 68.85% in the control group, the difference being significant(P<0.05). Compared with those before treatment,the ratios of normal electroencephalogram in the two groups were increased after treatment,and the ratio in the study group was higher than that in the control group, differences being significant(P<0.05). Compared with those before treatment, the scores of Hamilton Depression Scale(HAMD) in the two groups were decreased after treatment, and the score in the study group was lower than that in the control group,differences being significant(P<0.05);the levels of 5-hydroxyindoleacetic acid(5-HIAA) in plasm in the two groups were increased after treatment,and the level in the study group was higher than that in the control group, differences being significant(P<0.05). Conclusion: The therapy of scalp pointthrough-point electroacupuncture and body acupuncture combined with fluoxetine can effectively improve the clinical symptoms of patients with post-stroke depression and regulate the level of 5-HIAA,whose curative effect is better than that of simple fluoxetine.
