苯妥英钠对脑卒中后难治性癫痫患者神经功能缺陷及记忆的影响

观察负荷量苯妥英钠对脑卒中后难治性癫痫患者记忆及神经功能缺陷的影响。选择卒中后难治性癫痫患者55例,口服苯妥英钠。根据临床疗效分为有效组26例和效差组29例两个亚组。全组治疗后第3个月患者记忆商数均有下降趋势,而后数值有波动。有效组治疗第6个月恢复至治疗前水平,第9～12个月明显超过治疗前水平,效差组呈先升后降趋势。随访期间全组患者在治疗后第4个月神经缺陷功能逐渐改善,效差组未见明显变化,有效组在治疗后第3个月明显减轻。提示苯妥英钠可以改善中风后难治性癫痫患者的病情。

Effects of Sodium Phenytoin Monotherapy on the Neurologic Deficits and Menory of the Patients with Epilepsy Following Stroke

The effects of sodium pheytoin on the neurologic deficits and memory were studied in the patients with intractable epilepsy induced by stroke. There were 55 subjects with poststroke intractable epiepsy in the whole group, including 26 patients in wellcontrolled subgroup and 29 patients in bad-controlled subgroup. All patients received sodium phenytoin monotheraphy. In the whole group, memory scores decreased for short time and neurologic deficits increased gradually after treatment. In bad-controlled subgroup, a general trend of increase followed by decrease was found about memory performance, neurologic deficits showed few changes after treatment. In well-contorlled subgroup, the memmory scores were elevated to the levels of preteatment at 6th month and were significantly higher than that of pretratment over a period of 9th to 12the month after treatment, neurologic deficits scores got gradual improvement at and after the 3rd month during the 12-month study. Phenytoin can improve the condition of patients with intractable epilepsy following stroke.
