| 摘 要： | 作者报道了１９６５年６月～１９９４年１０月诊治７４例医源性胆管损伤的有关资料，并着重对胆囊切除术造成肝外胆管损伤的发病原因：（１）对胆囊切除术本身是一种充满潜在危险的手术认识不足；（２）术者操作粗疏，或其经验不成熟而又自负；（３）术者缺乏经验、技术水平低、缺乏应变能力、判断失误等进行探讨。作者就其处理方法予以讨论，认为术中发现胆管损伤应当即修复（修补或对端吻合），术后数天发现者、或多次修复重建手术失败者，最宜行规范的胆管空肠Ｒｏｕｘ－Ｙ吻合，均能获得满意效果

| Abstract: | patientswithiatrogenicinjuriesofbileductweretreatedfromJune1965toOctober1994inourhospital.Iatragenicextrahepaticbileductinjuriesmaybecausedbysomeerrorsduringchole-cystectomy.Theoperatordidnotrealizethatcholecystectomyitselfisaprocedurewithinherentandpo-tentialrisks.Theoperatorwascareless,rudeandarrogantinoperating.Theoperatorlackedexperienceandskil,andwasunabletodealwithdificultsituationduringoperation.Alsowediscussedthemanage-mentofbileductinjuries,andsuggestedthatintraoperativeinjuriesofextrahepaticbileductshouldbereconstructedimmediatelyincludingrepairoforendtoendanastomosesoftheinjuriesofbileduct.Cholecystojejunostomy(Roux-en-Ytechnique)isthebestreconstructionprocedureforpatientswiththeinjuriesofbileductafteroperationorfailureofrepetitionalrepairedandreconstructiveoperationsofbileduct.Satisfactoryresultcouldbeobtained. |
