肾性高血压患者血浆内皮素和降钙素含量对血液透析中血压的影响

目的　探讨血液透析前后及透析中血浆内皮素（ET-1）和降钙素（CGRP）含量变化对肾性高血压患者血压的影响。方法　18例血液透析（HD）中高血压病人和13例HD中无高血压患者分别在透前、透后及透析中测血压和血中ET和CGRP含量,20例健康成年人血中ET-1和CGRP作为对照。结果HD患者血浆ET-1明显高于正常健康组（P<0.05）,HD高血压患者ET-1含量明显高于非高血压组（P<0.05）,血透后 ET-1比血透前明显减少（P<0.01）,ET-1含量与血压变化呈正相关关系（r3=0.607,r0=0.587,P<0.01）。HD患者CGRP水平明显低于正常对照组（P<0.01）,HD高血压患者与非高血压患者之间显著差异（P<0.05）,高血压组的血压升高与CGRP含量下降呈明显负相关（r3=-0.73,r0=-0.69,P<0.01）非高血压组血压变化与CGRP下降之间无明显相关性（r3=0.19,r0=0.16,P>0.05）。结论 ET-1和CGRP平衡破坏是HD患者透析中高血压的原因之一。

Effects of ET-1 and CGRP Content on the Hemodialysis Blood Pressure in Patients with Renal Hypertension

Objective To investigate the effects of endothelin (ET-1) and calcitonin gene related peptide (CGRP) content before, during ad after hemodialysis (HD) on the blood pressure in patients with renal hypertension. Methods The subjective were divided into three groups: eighteen patients received HD with hypertension as hypertension group; thirteen patients received HD without hypertension as nonhypertension group, twenty healthy adults as control group. The blood pressure were taken and the levels of ET-1 and CGRP were measured by radioimmunoassy before, during and after HD. Results The ET-1 levels were significantly higher in patients received HD than that in normal controls (P <0. 05) and also significantly higher in patients with hypertension than that without hypertension (P <0. 05 ). The ET-1 levels in patients after HD were much lower than that before HD( (P <0. 01). There was a positive correlation between the levels of ET -1 and blood pressure (r3 =0. 607, r0=0. 587, P <0. 01) . The levels of CGRP were significantly lower in patients received HD than that in normal controls (P <0. 01).There was statistic difference in plasma CGRP levels between the HD with hypertension and without hypertension(P <0. 05) . There was a negative correlation between the levels of CGRP and blood pressure in patients with hypertension (r3 = -0.73, r0 = -0.69, P <0.01) . Nevertheless, there was no correlation between the levels of CGRP and blood pressure in patients without hypertension (r3 =0.19, r0 =0.16 , P> 0. 05).
