复方丹参联合小儿肺热咳喘口服液对重症肺炎患儿凝血指标、血小板参数及炎症因子的影响

目的研究复方丹参联合小儿肺热咳喘口服液对重症肺炎患儿凝血指标、血小板参数及炎症因子的影响。方法选取我院2012年6月至2015年6月重症肺炎患儿92例,抽签随机分为观察组与对照组两组,每组46例,给予对照组患儿常规西药治疗,观察组患者在对照组治疗基础上给予复方丹参联合小儿肺热咳喘口服液治疗。比较两组患者治疗有效率,凝血酶时间(TT)、凝血酶原时间(PT)、活化部分凝血活酶时间(aPTT)、纤维蛋白原(Fbg)、抗凝血酶(AT)和D二聚体(D-D)等凝血指标变化情况,血小板计数(PLT)、平均血小板体积(MPV)等血小板参数变化情况,比较两组治疗前后肿瘤坏死因子(TNF-α)、白介素-6(IL-6)、白介素-8(IL-8)、C反应蛋白(CRP)等炎性因子水平变化。结果观察组患者临床治疗总有效率为91.30%较对照组73.91%显著较高(P0.05);观察组患者TT、PT、aPTT、D-D水平较对照组低(P0.05),Fbg、AT水平较对照组高(P0.05);观察组患者PLT、MPV参数水平较对照组低(P0.05);观察组患者治疗后TNF-α、IL-6、IL-8、CRP等炎性指标水平较对照组低(P0.05)。结论复方丹参与小儿肺热咳喘口服液联合治疗能提高重症肺炎患儿的治疗有效率,患儿的凝血功能得到改善,血小板参数恢复正常,炎症因子水平降低。

Effects of Compound Danshen Combined with Xiaoer Feire Kechuan oral Liquid on Coagulation Parameters,Platelet Parameters and Inflammatory Factors in Children with Severe Pneumonia

Objective To study the effects of Compound Danshen combined with Xiaoer Feire Kechuan Oral Liquid on coagulation parameters,platelet parameters and inflammatory factors in children with severe pneumonia.Methods 92 cases children with severe pneumonia treated in our hospital between June 2012 and June 2015 were randomized into observation group and control group,46 cases in each.The control group was treated with routine western medicine.On the basis,the observation group was treated with Compound Danshen combined with Xiaoer Feire Kechuan oral liquid.The response rate,changes of thrombin time (TT),prothrombin time (PT),activated partial thromboplastin time (aPTT),fibrinogen (Fbg),antithrombin(AT),D-dimer(D-D) and other blood coagulation parameters,changes of platelet count(PLT),mean platelet volume(MPV) and platelet parameters were compared between the two groups.The changes of levels of tumor necrosis factor (TNF-α),interleukin 6 (IL-6),interleukin-8 (IL-8),C-reactive protein (CRP) and other inflammatory factors before and after treatment were also compared.Results The total response rate in observation group(91.30％)was significantly higher than that in control group(73.91％) (P ＜ 0.05);The levels of TT,PT,aPTT and D-D in the observation group were lower than those in the control group (P ＜ 0.05)while the levels of Fbg and AT were higher than those in the control group(P ＜ 0.05);Compared with the control group,the levels of PLT and MPV in the observation group were lower(P ＜ 0.05);After treatment,the levels of TNF-α,IL-6,IL-8,CRP and other inflammatory factors in the observation group were lower(P ＜ 0.05).Conclusion Compound Danshen combined with Xiaoer Feire Kechuan oral liquid can improve the response rate of treatment in children with severe pneumonia.It can improve coagulation function,normalize platelet parameters and reduce levels of inflammatory factors.
