新生儿缺氧缺血性脑损伤的凝血变化特点及早期干预价值

摘    要:目的分析新生儿缺氧缺血性脑损伤的凝血变化特点及早期干预价值。方法选取我院2011年10月至2015年8月缺氧缺血性脑损伤新生儿200例,随机分为观察组与对照组两组,每组100例,将实施早期干预的患儿设置为干预组,不实施早期干预的患儿设置为对照组。再选取同期出生的健康新生儿50例,比较缺氧缺血性脑损伤新生儿与健康新生儿凝血变化特点,比较干预组与对照组两组患儿的智力指数(MI)和发育商评分(DQ)。结果缺氧缺血性脑损伤新生儿PLT、Fbg水平较健康新生儿显著较低(P0.05),PT、aPTT、D-D水平较健康新生儿显著较高(P0.05);干预组新生儿的MI评分较对照组显著较高(P0.05);干预组新生儿DQ评分较对照组显著较高(P0.05)。结论患有缺氧缺血性脑损伤的新生儿凝血功能会受到影响,实施早期干预能改善患儿预后,降低脑瘫及智力发育落后的发生率。

Abstract:Objective To analyze characteristics of coagulation changes of neonatus with hypoxicischemic brain damage and early intervention value.Methods 200 neonatus with hypoxic-ischemic brain damage in our hospital from October 2011 to August 2015 were selected,they were divided by random lot method into the observation group and the control group (each 100 cases),neonatus who received early intervention were marked as intervention group,neonatus without early intervention were marked as control group.50 healthy neonatus born in the same period were selected,characteristics of coagulation changes of neonatus with hypoxic-ischemic brain damage and healthy neonatus were compared,intelligence indexes(MI) and development scores (DQ) in intervention group and control group were compared.Results PLT,Fbg levels in neonatus with hypoxic-ischemic brain damage were significantly lower than healthy neonatus (P < 0.05),PT,aPTT,D-D level were significantly higher than healthy neonatus (P < 0.05);MI scores in intervention group were significantly higher than control group (P < 0.05);DQ scores in intervention group were significantly higher than control group (P < 0.05).Conclusion Coagulation function of neonatus with hypoxic-ischemic brain damage will be affected,early intervention can improve prognosis of children,reduce incidence of cerebral palsy and mental retardation.
