原发性高血压患者血同型半胱氨酸、颈动脉粥样硬化与24小时血压变异性的关系

摘    要:目的探讨原发性高血压患者血同型半胱氨酸(Hcy)、颈动脉粥样硬化与24小时血压变异性(BPV)(收缩压、舒张压)之间的关系。方法选取我院原发性高血压患者120例,根据血Hcy水平的不同,分为3组:A组36例(Hcy15.0μmol/L),B组62例(15.0μmol/L≤Hcy20.0μmol/L),C组22例(≥20μmol/L),分别测定其颈动脉厚度(IMT)及24小时收缩压及舒张压、脉压标准差(SD)。结果三组中血压变异性随着HCY的增高,变异性增大,IMT厚度也随之增加,组间比较差异有统计学意义(P0.05);相关性分析提示:血Hcy水平与24小时BPV(收缩压、舒张压)之间存在相关性(r=0.570、0.512,P0.01);IMT与24小时BPV呈直线相关(r=0.653、0.635,P0.01),IMT与Hcy水平也呈直线相关(r=0.1252,P0.01)。结论原发性高血压患者血压变异性增高可能与高Hcy、IMT相关,Hcy增高可能造成动脉粥样硬化的重要因素。

The Relationship between Hcy and Carotid Atherosclerosis in Patients with Essential Hypertension and Blood Pressure Variability in 24 Hours
Abstract:Objective To investigate the relationship between Hcy , carotid atherosclerosis and blood pressure variability ( BPV ) ( systolic blood pressure and diastolic blood pressure ) in patients with essential hypertension .Methods 120 cases of hypertensive patients , according to the different levels of Hcy , divided into 3 groups:36 cases in group A(Hcy <15.0 μmol/L),62 cases of group B(15 μmol/L=Hcy <20.0μmol/L),C group(22 cases of more than 20μmol/L),three groups were measured IMT and 24 hour systolic blood pressure and diastolic blood pressure ,pulse pressure standard deviation ( SD) .Results The variation of blood pressure in the three groups increased with the increase of HCY ,and the variation of blood pressure was increased ,and the thickness of IMT was increased , the difference was statistically significant ( P <0.05 );Correlation analysis:correlation between Hcy level and 24 h BPV ( systolic blood pressure , diastolic blood pressure)(r=0.570,0.512,P<0.01);IMT was linearly correlated with 24 h BPV(r=0.653,0.635,P<0.01);IMT and Hcy level also showed a linear correlation (r=0.1252 R,P<0.01).Conclusion Increased blood pressure variability in patients with essential hypertension may be associated with high Hcy and IMT , and increased Hcy may be an important factor in atherosclerosis .
