局部注射泡沫硬化剂治疗下肢静脉血栓症的大样本临床调研

目的探讨局部注射泡沫硬化剂治疗下肢静脉血栓症的疗效以及对凝血状态的影响效应。方法选取确诊的下肢静脉血栓患者为研究对象,对照组患者300例,进行常规治疗。泡沫硬化剂治疗组患者285例。比较两组局部注射泡沫硬化剂治疗下肢静脉血栓症的疗效以及对凝血状态的影响。结果泡沫硬化剂治疗患者治疗前膝上20厘米周径差为6.61±1.23厘米,治疗后显著减低至2.42±1.14厘米;膝下15厘米周径差也有治疗前的4.74±2.06厘米降至治疗后的2.21±0.99厘米,且低于常规治疗对照组治疗后的水平(P0.05)。治疗14 d后统计治疗效果情况,泡沫硬化剂治疗组显效率为71.93%,总有效率达到98.25%;泡沫硬化剂治疗组的显效率和总有效率均高于对照组,无效率显著低于对照组(P0.05)。比较两组凝血状态情况,除a PTT在两组治疗前后未见明显改变外,Fbg水平、PLT、PT以及TF均有改善,差别有统计学显著性(P0.05)。此外,治疗后Fbg和TF状态在泡沫硬化剂组明显优于对照组(P0.05)。结论局部注射泡沫硬化剂治疗下肢静脉血栓症具有一定优势,且能改善凝血功能。

A Large Sample Clinical Investigation for the Effect of Foam Sclerotherapy Among Venous Thrombosis of Lower Limbs Patients

Objective To investigate the effect of local injection of foam sclerosing agent on the treatment of venous thrombosis of lower extremities and its influence on coagulation status.Methods 300 patients with venous thrombosis of lower extremity were selected and treated with routine therapy.There were 285 patients in the foam sclerotherapy group.Comparison between the efficacy of two groups of local injection of foam sclerotherapy in the treatment of venous thrombosis of the lower limbs was analyzed and the effect on coagulation status was discussed also.Results The difference of 20 cm circumference was 6.61 ± 1.23 cm in treatment group and 2.42 ± 1.14 cm after treatment.The difference of 15 cm circumference was 4.74 ±2.06 cm in treatment group and decreased to 2.21 ±0.99 cm after the treatment,and the level in treatment group was lower than the level of the controls(P ＜ 0.05).The effective rate was 71.93％ and general effective was as high as 98.25％ in the foam sclerotherapy group,higher than the controls significantly (P ＜ 0.05).The levels of Fbg,PLT,PT and TF were improved,but the difference was statistically significant(P ＜ 0.05),except for aPTT in the two groups before and after treatment,no significant change was observed.In addition,after treatment Fbg and TF status in the foam sclerosis group was significantly better than the control group (P ＜ 0.05).Conclusion Injection of foam sclerosing agent in the treatment of venous thrombosis has certain advantages,and to improve the clotting state of a good effect.
