严重外伤性虎口挛缩的显微外科治疗

目的 研究严重外伤性虎口挛缩的显微外科治疗效果。方法 应用组织瓣移位或移植的方法修复严重外伤性虎口挛缩并经扩大矫形。结果 27例皮瓣全部成活，虎口外形良好，可完成拇指外展对掌功能。结论 虎口严重挛缩经扩大后组织缺损不能用断层皮片或“Z”字成形修复者，应选用显微外科方法修复，疗程短，疗效好；能应用局部皮瓣转位者不用游离移植，其血管蒂能用非主干血管可不用主干血管。

Microsurgical Treatment for Severe Traumatic Contracture of First Web

Objective Microsurgical therapeutic effect of severe traumatic contracture of first web was studied retrospectively.Methods The authors made a follow-up for 27 patients with the disease who received large plastic and transpositon of transplantation of tissue flap. Results The a bluction flaps all survived.The appearance of first web was satisfactory.The abduction function of thumb improved.Conclusion Microsurgical methods should be selected for the severe contracture of first web when free skin graft or Z plasty wasn't suitalbe for the disease because of the tissue defect after being enlarged.Its effect was good and therapeutic period was short.As for the choice of the methods, regional flap is prior to free graft and flap pedicle with non major vessels is prior to that with major vessels.
