摘    要:目的:观察经皮耳迷走神经电刺激联合经颅直流电刺激对脑卒中恢复期患者上肢功能的影响。方法:选取44例脑卒中患者随机分为对照组和观察组各22例。2组患者均进行药物治疗、常规康复治疗和经颅直流电刺激(tDCS)治疗,观察组患者在此基础上行经皮耳迷走神经刺激(taVNS)。2组患者均于治疗前、治疗3周后检测皮质潜伏期(CL)和中枢运动传导时间(CMCT),并采用Fugl-Meyer运动评定量表上肢部分(FMA-UE)和改良Barthel量表(MBI)评估患者的上肢运动功能和日常生活活动能力。结果:治疗3周后,2组患者CL、CMCT较治疗前减少(均P<0.05),且观察组低于对照组(均P<0.05);FMA-UE、MBI评分均较治疗前增加(均P<0.05),且观察组高于对照组(均P<0.05)。结论:taVNS联合tDCS治疗能促进脑卒中偏瘫患者上肢功能的恢复,提高患者的日常生活能力。

Transcutaneous auricular vagus nerve stimulation combined with transcranial direct current stimulation on upper limb function during convalescence of stroke
Abstract:Objective: To observe the effect of transcutaneous auricular vagus nerve stimulation (taVNS) combined with transcranial direct current stimulation (tDCS) on upper limb function during stroke recovery. Methods: From 2022 to 2023, 44 patients with stroke who were hospitalized in the Department of Rehabilitation Medicine, The Second Affiliated Hospital of Dalian Medical University were selected. According to the envelope method, the patients were randomly divided into control group (n=22) and observation group (n=22). Both groups received conventional drugs, rehabilitation therapy and tDCS treatment, and those in the observation group received taVNS on the basis of conventional treatment. CL and CMCT were measured before and 3 weeks after treatment, and evaluated by FMA-UE and MBI. Results: All patients completed rehabilitation training, and no adverse events occurred. After 3 weeks of treatment, the scores of CL, CMCT, FMA-UE and MBI in the two groups were significantly improved as compared with those before treatment (P<0.05), and those in the observation group were significantly better than those in the control group (P<0.05). Conclusion: The taVNS combined with tDCS can promote the recovery of upper limb function in stroke patients with hemiplegia, and improve the ability of daily living.
