摘    要:目的 探讨急性心肌梗死 ( AMI)后外周血单核细胞中血管内皮生长因子 ( VEGF)的表达及其与 AMI左心室收缩功能的关系。方法 分别抽取 2 5例行急诊经皮冠状动脉介入 ( PCI)治疗的 AMI患者发病后第 1、5、10和15 d的外周静脉血 ,分离、培养单核细胞 ,用酶联免疫吸附法 ( ELISA)测定 VEGF浓度。结果 单核细胞分泌的VEGF在 AMI后第 5 d即达高峰 ,显著高于对照组 ( 3 43 .2± 82 .5 pg/ml vs 14 3 .3± 2 4.2 pg/m l,P<0 .0 5 )。单核细胞分泌的 VEGF峰植与肌酸磷酸激酶 ( CK)峰值无显著相关。 PCI术后左心室收缩功能改善的 AMI患者 ,其单核细胞分泌的 VEGF水平显著高于左心室收缩功能未改善者 ( 867.6± 113 .1pg/ml vs2 3 4.8± 82 .4pg/ml,P<0 .0 5 )。结论 AMI患者 PCI术后单核细胞产生的 VEGF与左心室收缩功能改善有关

Abstract:Objective To investigate expression of vascular endothelial growth factor (VEGF) by peripheral blood mononuclear cells (PBMCs) in acute myocardial infarction (AMI), and the relationship between VEGF and left ventricular systolic function in AMI. Methods 25 patients with AMI and 12 controls were observated for this study. PBMCs were isolated from peripheral blood on days 1, 5, 10 and15 after acute percutaneous coronary intervention (PCI) of AMI and cultured for 24h.VEGF levels were measured by enzyme linked immunosorbant assay (ELISA). Results VEGF levels in the cultured medium of PBMCs after incubation of 24h (PBMC VEGF) were elevated gradually and reached a peak on day 5, which was significantly higher than that of control (343.2±82.5pg/ml vs 143.3±24.2pg/ml, P <0.05). Peak PBMC VEGF levels showed no correlation with peak CK levels. PBMC VEGF levels in patients with improvement of left ventricular systolic function during the course of AMI after acute PCI were significantly higher than that in patients without improvements.Conclusions PBMC VEGF played an important role in the improvement of left ventricular systolic function after AMI with acute PCI.
