169例早产的临床分析

目的: 探讨早产的原因及影响早产儿预后的相关因素。方法: 对 2年间 169例早产患者的临床资料进行分析并随机抽取足月分娩 30例进行有关对照。结果: 胎膜早破、妊高征、妊娠期胆汁淤积症、多胎妊娠及产前出血占据早产病因的前5位; 不明原因的自然早产者中性粒细胞百分比明显高于足月产 (P<0 05); 妊高征和产前出血是医源性早产的主要原因; 早产儿病率与孕周、体重呈负相关 (P<0 005), 产前皮质激素运用的次数明显地改善了新生儿的预后 (P<0 05)。结论: 早产病因复杂, 是导致围产儿病率和围产儿死亡的最主要因素。提高早产的预防、诊断及治疗水平, 对围产医学质量的提高有着十分重要的意义。

Clinical analysis on 169 cases of premature labor

Objective:To explore the causes of premature labor and the related factors to prognosis of the premature Methods:The clinical data of 169 patients with premature labor within last two years were analyzed and compared to that in 30 cases with full-term delivery randomly selected Results:The preceding causes of premature labor were premature rupture of membranes (PROM), pregnant-induced hypertension (PIH), intrahepatic cholestasis of pregnancy (ICP), multiple pregnancies and prenatal hemorrhage The percentage of neutrophilic granulocyte in spontaneous premature labor for unexplained causes was obviously higher than that in full-term delivery ( P <0 05) The main causes for iatrogenic premature labor were PIH and prenatal hemorrhage The morbidity of the premature had negative correlation with the gestational age and birth weight ( P <0 005) The times of antenatal corticosteroids significantly improved the newborns' prognosis ( P <0 05) Conclusion:The complicated causes for premature labor should be the most important factors resulting in morbidity and mortality of perinatal neonates Enhancing the preventive, diagnosis and treatment level of premature labor play an important role in promoting the perinatal medical quality
