股静脉与颈内静脉半永久双腔导管在血液透析中的应用

目的:比较股静脉与颈内静脉半永久双腔导管在血液透析中的使用情况。方法:采用前瞻性研究,将50例不能建立动静脉内瘘的患者分为两组:颈内静脉组31例,将半永久性双腔导管留置于颈内静脉;股静脉组19例,将半永久性双腔导管留置于股静脉。观察两组患者导管使用寿命、并发症、透析血流量及Kt/V值等指标。结果:颈内静脉组导管使用寿命为(387±101)天,而股静脉组为(210±88)天,差异有显著性(P<0.05);两组的透析血流量与Kt/V值差异无显著性。结论:对不能建立动静脉内瘘的患者,股静脉及颈内静脉半永久性双腔导管是较好的血管通路,颈内静脉优于股静脉。

Application of cuffed-tunneled femoral and internal jugular vein catheters in hemodialysis

Objective:To campare the advantages and disadvantages of cuffed-tuneled femoral vein catheter with those of internal jugular vein.Methods:In a prospective study we inserted 50 cuffed-tuneled catheters in 50 patients undergoing hemodialysis,31 catheters in internal jugular vein as jugular group and the other 19 in femoral vein as femoral group.The catheter survival,related complications,mean blood flow during hemodialysis and Kt/V in the two groups were observed.Results: The suivival of jugular group was 387 days,longer than that of femoral group (210 days)obviously,and the infective and occlusive rates were less.These were no differences in the mean blood flow during hemodialysis and Kt/V between the two groups.Conclusion:The cuffed-tnneled femoral and internal jugular vein catheters are supplement of permater vascular access and the former should be the first choice.
