住院痴呆患者照料者心理健康状况及相关因素研究

目的探讨住院痴呆患者照料者的心理健康状况及其相关因素。方法采用症状自评量表（SCL-90）作为评定工具，对住院痴呆患者照料者的心理健康状况进行调查。结果（1）住院痴呆患者照料者的SCL-90总分及抑郁因子分与国内常模比较，差异无统计学意义（P〉0.05）：躯体化因子分显著高于常模，其他因子评分显著低于国内常模，差异有统计学意义（P〈0．01）。（2）SCL．90总分显示，女性、高龄、受教育水平低、身体健康状况差、收入较低、照料人数多、照料时间长的照料者心理健康状况较差，差异有统计学意义（P〈0．05）。（3）男性、低年龄、病程长、痴呆程度较严重、躯体疾病较多对照料者心理健康影响更明显，差异有统计学意义（P〈0.01）。（4）照料者年龄、文化水平、过去经历、身体状况、亲子关系、照料时间及患者病程进入回归方程．决定系数为0．599。结论住院痴呆患者照料者心理健康受多方面因素的影响．要提高照料者的心理健康水平，必须从多个方面综合考虑并采取措施。

Research of the status of mental health and the influence factors on caregivers of the hospitalized dementia

Abstract] Objective To investigate the mental health status and influence factors on caregivers of the hospitalized de- mentia. Methods Symptom Check List-90 （SCL-90） was applied as evaluation tool to investigate the mental health status of care- givers for patients with dementia in the hospital. Results （ 1 ） Comparing with the domestic norm, the difference on the total score of SCL-90 and score of depression factor of the caregivers for hospitalized patients with dementia had no statistical significance （P〉0.05） ;while the score of somatization factor was remarkable higher than the norm,and the scores of the other factors were prominently higher than the norm with statistically significance （P〈0.01）. （2）The total score of SCL-90 showed that the caregivers of female, advanced age, low educational level, worse health status,lower income,large number of patients or care for a long time were with poor mental health status, and the difference had significant difference （P〈0.05）. （3）The young male patients with long course of disease and serious degree of dementia had more obvious influence on the mental health status of the caregivers with sta tistically significant difference （P〈0.01）. （4）The caregivers＇ age, educational level, past experiences, physical status, parent-child relationship, care time and course of disease were entered into the regression equation with the determination coefficient of 0.599. Conclusion The mental health status of the care givers for patients with dementia are affected by many factors, so it must take many factors into consideration and adopt measures to improve the level of mental health of the caregivers.
