手术前新辅助化疗联合放射治疗治疗局部晚期宫颈癌的临床分析

目的探讨手术前新辅助化疗联合放射治疗治疗局部晚期宫颈癌的疗效。方法分析35例Ⅰb2期～Ⅱb2期术前新辅助化疗联合放射治疗（观察组）与32例单纯手术（对照组）的局部晚期宫颈癌患者的临床资料,评价观察组和对照组患者的近期疗效,并比较两组患者术中出血量、手术时间、术后病理特征与疗效。结果观察组患者治疗总有效率为80.9%,高于对照组的9.3%（P〈0.05）。两组患者手术时间、术中出血量、脉管受侵以及阴道切缘癌残留率比较,差异无统计学意义（P〉0.05）,观察组患者宫旁浸润率及淋巴结转移率低于手术组（P〈0.05）。结论手术前新辅助化疗联合放射治疗可以缩小局部肿瘤,使宫旁浸润消退,可提高局部晚期宫颈癌的疗效。

Clinical analysis of neoadjuvant chemotherapy plus radiotherapy before operation on locally advanced cervical cancer

Objective To analyze clinical effect of neoadjuvant chemotherapy（ NC） plus radiotherapy before operation on locally advanced cervical cancer. Methods 35 cases received Ⅰb2 ～ Ⅱb2 of NC before operation combined with radiotherapy（ observation group） and 32 cases surgery alone（ control group）. Clinical information were compared,and short-term efficacy,operative blood loss,operative time and postoperative pathological features were evaluated. Results The total effective rate of observation group was 80. 0%,higher than 9. 3% of control group（ P〈0. 05）. There were no significant differences on operative time,blood loss,vascular invasion,and vaginal cancer stump rate between two groups（ P〈0. 05）. Parametrial infiltration rate and lymph node metastasis rate of observation group was lower that those of control group（ P〈0. 05）. Conclusions NC before operation combined with radiotherapy can shrink local tumor,regress parametrial infiltration,and improve the clinical efficacy for the disease.
