氨基酮戊酸光动力疗法治疗90例皮肤癌及癌前病变的临床观察

摘    要:目的探讨氨基酮戊酸光动力疗法(ALA—PDT)治疗皮肤癌及癌前病变的临床价值。方法选取收治的90例皮肤癌患者行ALA—PDT治疗,观察评估其治疗效果和不良反应。结果治疗6个月后,完全反应67例(74.4%),部分反应17例(18.9%),无效6例(6.7%),复发13例(14.4%)。基底细胞癌、鳞状细胞癌、疣状癌、日光性角化、Bowen病、Paget病等皮肤癌及癌前病变的有效率和复发率分别为92.9%和14.3%、92.0%和16.0%、100%和14.3%、100%和12.5%、90.0%和10.0%、91.7%和16.7%,各疾病类型之间的有效率和复发率差异均无统计学意义(P〉0.05),且均未出现严重不良反应。结论ALA—PDT治疗皮肤癌及其癌前病变的效果满意、不良反应轻。

Abstract:Objective To investigate the clinical value of ALA-PDT in the treatment of skin cancer and precancerous lesions. Methods The 90 cases of patients admitted to hospital were treated with ALA- PDT treatment. The therapeutic effect and adverse reactions were observed and compared. Results After treatment 6 months,complete response in 67 cases (74.4%) ,partial response in 17 patients ( 18.9% ) ,ineffective in 6 cases (6. 7% ) ,recurrence in 13 cases (14. 4% ). The total efficiency rate and recurrence rate of basal cell carcinoma, squamous cell carcinoma, verrucous carcinoma, solar keratoses, Bowen's disease, Paget's disease were 92. 9% and 14. 3% ,92. 0% and 16. 0% ,100% and 14. 3% ,100% and 12. 5% ,90. 0% and 10. 0% ,91.7% and 16. 7%. The total efficiency rate and recurrence rate between each type of disease was no significant difference (P 〉 0. 05) ; and there was no serious adverse reactions. Conclusions ALA- PDT has satisfactory effects in the treatment of skin cancer and precancerous lesions with lighter adverse reactions.
