早期微量喂养联合新生儿抚触对早产儿喂养难关的疗效分析

目的：探究早期微量喂养结合新生儿抚触对早产儿喂养难关的治疗效果。方法：将75例喂养不耐受的早产儿随机分为治疗组38例，给予早期微量喂养与新生儿抚触；对照组37例，给予常规微量喂养。结果：治疗组发生1例坏死性小肠结肠炎，而对照组有4例发生了坏死性小肠结肠炎，2例发生了肠穿孔，3例发生了吸人性肺炎。治疗组在恢复至出生体重时间、达全量肠内营养时间、住院天数、胎粪排出时间较对照组明显降低（P〈O．05）。而蓝光照射的时间（第1天TCB〉6mg／dl、第2天TCB〉9mg／dl、第3天后TCB〉12mg／dl时则予光疗）两组没有统计学意义。结论：微量喂养结合新生儿抚触可促进早产儿胃肠道发育成熟，减少可能发生的喂养不耐受问题，并且可促进胎粪排出，改善患儿营养状况，缩短住院天数，减少并发症；而对黄疸水平没有明显的影响。

Therapeutic Effect of the Early Minimal Feeding and Stroking in Premature Infants with Feeding Intolerance

Objective：To explore the therapeutic effect of the Minimal feeding and stroking in premature infants with feeding intolerance. Methods： 75 cases of the premature infants with feeding intolerance were randomly divided into the two groups. The treatment group（38 cases of neonatal） were given by the Minimal feeding and stroking. And the con- trol group（37 cases） were given the routine Minimal feeding. Results：The treatment goup only had one severe complica- tions that is the necrotized enterocolitis. While the control group had 4 cases of necrotizing enterocolitis, 2 cases of in- testinal perforation,3 cases of aspiration pneumonia. The time to establish complete enteral nutrition, days to regain birth weight length of stay and the time to excrete meconium were significantly shorter in the treatment group than those in the control group（P~0. 05）. But the study had not achieve statistical significance in time of blue light irradia- tion（first day TCB~6mg/dl, second day TCB~9mg/dl, after third day TCB~12mg/dl, then need phototherapy）. Con- clusion：The Minimal feeding and stroking can promote gastrointestinal developing ripeness, lessen the probable of feed- ing intolerance, to promote the discharge of meconium, improve nutritional state, shortened the hospital stays, reduc- ing complication; while the jaundice levels had no obvious impacts.
