儿童Volkmann缺血性挛缩晚期治疗的探讨

目的；探讨儿童Ｖｏｌｋｍａｎｎ缺血性挛缩晚期治疗的方法。方法：采用前臂屈肌腱延长加软组织松解术治疗儿童Ｖｏｌｋｍａｎｎ缺血性挛缩２６例，其中：２０例将屈指深、浅肌腱分别行“Ｚ”形延长吻合；６例行深、浅屈指肌腱交错吻合，两种术式后均将手指腕关节过伸位固定２周。结果：２６例患儿中优１０例（３８.４６％），良９例（３４．６１％），可４例（１５．３８％），差３例（１１．５４％）。结论：采用前臂屈肌腱延长加软组织松解术治疗儿童晚期Ｖｏｌｋｍａｎｎ缺血性挛缩，除恢复了手的外观，改善和重建了手的部分功能，也解除了大龄儿童手畸形后的心理障碍。

Discussion About Treatment of Late Stage in Children Ischemic Contracture of Volkmann

Objective:Methods was anayzed in treatment of late stage in children ischemic contracture of Volkmann.Methods: 26 cases of ischemic contracture were treated with lysis of soft tissue and flexor digitorm lengthening, 20 patients ofthese cases were operated with superficial flexor and deep ones"Z"plastic, 6 patients of those were treated with cross-anas-tomesis,arthrodesis of interphalangeal joint of hands and wrist were used in hyper - traction for two weeks after two operationprograms.Result:The results were excellent in 10 cases(38.46% ), fine in 9 cases(34. 61% ), approve in 4 cases(15. 38% ), bad in 3 cases(11. 54% ). Conclusion: This operation programs recover apperances of hand and rebuild part offunction of hand and removed the psychogenic obstacle of older children at the same time.
