大面积肺动脉栓塞不同药物溶栓前后同型半胱氨酸及白细胞介素-8浓度的变化

 目的 观察同型半胱氨酸( homocysetine,Hcy)、白细胞介素-8(interleukin -8,IL -8)浓度在大面积肺动脉栓塞患者溶栓前后的变化.方法 大面积肺动脉栓塞88例,随机分为瑞替普酶组和尿激酶组进行溶栓治疗,每组44例,同时选择40位健康人作为对照组,观察患者溶栓前后及健康对照组血清中Hcy、IL -8的浓度变化.结果 (1)大面积肺动脉栓塞患者较健康对照组血清中Hcy、IL-8浓度明显升高,两组比较差异有统计学意义(P＜0.05)；(2)瑞替普酶组和尿激酶组溶栓后Hcy浓度均降低,瑞替普酶组溶栓前后比较差异有统计学意义(P＜0.05)；(3)瑞替普酶组和尿激酶组溶栓后IL -8浓度均升高,瑞替普酶组溶栓前后比较差异有统计学意义(P＜0.05).结论 瑞替普酶能快速降低肺动脉栓塞的危险程度；IL -8介导的炎性反应可以造成急性肺损伤,溶栓后需要继续抗炎治疗.
